COVID-19 Coronavirus Office Procedures

Your health and safety is very important to us. In response to the COVID-19 pandemic, we have implemented new office procedures to reduce the risk of exposure and spread of infection.

Only patients, caregivers and guardians (for minors and adults) will be permitted in the building. Other family members and guests will be asked to wait outside the building or in their vehicle.

We ask that everyone wear a mask upon entering our offices. If you do not have a mask, one will be given to you.

Well Clinic – Main Entrance 

  • Well, preventative visits pediatric & adult/immunizations
  • Chronic disease management
  • Prenatal patients
  • Women’s health, family planning, FREE pregnancy testing

Sick Clinic – Door 2

  • Sick visits pediatric & adult
  • COVID-19 testing if provider deems necessary

COVID-19 Testing Information

Patients must make an appointment with one of our providers at our East Paulding Road sick clinic to determine if a test is needed.

You do not need to be a current patient to make an appointment with one of our providers.

To make an appointment for an in-person assessment and for testing, contact us at 260-458-2641.

We accept Medicaid, Medicare, and other insurance plans. If you do not have insurance, we offer a sliding fee discount.

Both offices are open during normal business hours. Please try to arrive early as it may take longer to enter the building due to the new precautionary measures.
